Challenges Encountered in Schema Therapy for Eating Disorders

Öz

Eating disorders and their associated maladaptive coping mechanisms can be likened to an old lifebuoy keeping an individual afloat in a stormy sea. Initially, this lifebuoy prevents the individual from sinking emotionally; it makes survival possible during moments of intense anxiety, loneliness, or helplessness. However, over time, this lifebuoy hardens, restricts movement, and makes it difficult for the individual to swim towards the shore. While the lifebuoy still keeps them afloat, it no longer helps them reach a safe place, but rather keeps them stuck in the same spot. At this point, change requires temporarily letting go of the lifebuoy and learning to swim in the water, relying on one's own body. During this process, the individual may experience vulnerability, fear, and uncertainty in the short term, as they are giving up their accustomed protective tool. Schema therapy provides a safe therapeutic context where the individual can emotionally regulate themselves and experience new coping patterns during this transitional phase of the change process. The therapeutic relationship allows the client to be emotionally regulated and supported as they try out new and more functional coping mechanisms. In this context, the fundamental question is to what extent the individual's current coping mechanism is still functional and how willing they are to abandon it and try more flexible alternatives. Schema therapy aims to clarify the distinction between short-term and long-term well-being by supporting the client in making this assessment from a healthy adult perspective. Thus, moving away from eating disorder symptoms is considered not merely a behavioral change, but a transition to a safer and more sustainable life. In this context, the aim of this study is to provide information on selecting appropriate motivational interventions for the client's stage in the change process and how the modes leading to hesitation can be understood within the conceptual framework of schema therapy.

Öz

Yeme bozuklukları ve onlarla ilişkili uyum bozucu başa çıkma modları, fırtınalı denizde bireyi ayakta tutan eski bir can simidine benzetilebilir. Başlangıçta bu can simidi, bireyin duygusal olarak batmasını engeller; yoğun kaygı, yalnızlık ya da çaresizlik anlarında hayatta kalmayı mümkün kılar. Ancak zamanla bu simit sertleşir, hareket etmeyi kısıtlar ve bireyin kıyıya doğru yüzmesini zorlaştırır. Can simidi hâlâ yüzeyde kalmayı sağlasa da artık güvenli bir yere ulaşmayı değil, aynı noktada kalmayı sürdürmektedir. Bu noktada değişim, can simidini bir süreliğine bırakmayı ve suyun içinde kendi bedenine güvenerek yüzmeyi öğrenmeyi gerektirir. Bu süreçte birey, kısa vadede savunmasızlık, korku ve belirsizlik yaşayabilir; zira alışık olduğu koruyucu araçtan vazgeçmektedir. Şema terapi süreci, değişim sürecinin bu geçiş aşamasında bireyin duygusal olarak regüle olabildiği ve yeni başa çıkma örüntülerini deneyimleyebildiği güvenli bir terapötik bağlam sunmaktadır. Terapötik ilişki, danışanın yeni ve daha işlevsel başa çıkma yollarını denerken duygusal olarak düzenlenmesine ve desteklenmesine olanak tanır. Bu bağlamda temel soru, bireyin mevcut baş etme yolunun hâlâ ne ölçüde işlevsel olduğu ve onu bırakıp daha esnek alternatifleri denemeye ne kadar hazır olduğudur. Şema terapi, danışanın bu değerlendirmeyi sağlıklı yetişkin modundan yapabilmesini destekleyerek, kısa ve uzun vadeli iyilik hâli arasındaki ayrımı netleştirmeyi amaçlamaktadır. Böylece yeme bozukluğu belirtilerinden uzaklaşma, yalnızca bir davranış değişikliği değil, daha güvenli ve sürdürülebilir yaşama geçiş olarak ele alınmaktadır. Bu bağlamda bu çalışmanın amacı, danışanın değişim sürecindeki aşamasına yönelik uygun motivasyon artırıcı müdahaleleri seçmeye ve kararsızlığa yol açan modların şema terapideki kavramsal çerçevede nasıl anlaşılabileceğine dair bilgi sunmaktır.
